Please note that our application is brief, but is an important part of helping us match pets with families.
We suggest that you have the following information available before you begin:

• The name AND phone number of your vet (if a former pet owner within the past 5 yrs).
• Approval from your landlord if you rent (you may apply if you are still awaiting a response from your landlord).
• Approval from patrician management if you live on military base housing 
  
  

• This adoption application is not a contract securing adoption of any pet.
• The questions contained within this application are not meant for the sole purpose of excluding an applicant.
  Rather, it is one of several means of assisting PAS in identifying the best match for each applicant and animal. 
• PAS reserves the right to refuse adoption to any applicant.
  
  
  PAS does not share any information submitted in an application without the consent of the applicant, except for
  in cases where information is found to be falsified and/or which PAS deems to put at risk one of our animals or
  animals in other rescues or animal shelters. If you do not agree with this, or intend to knowingly falsify information in
